Completion of a Credit Card Authorization Form providing billing and payment information is required prior to the first session and will be kept on file and charged at the time of service.

Payment for your first session will be collected from you at the time of service & your insurance company will be billed.

After your EOB (Explanation of Benefits) & insurance payment are received, any remaining balance will be your responsibility or your account will be credited for any overpayment.

Serenity Counseling Services, LLC does NOT bill out-of-network insurance.

Depending on your plan, some insurance companies will reimburse a percentage of the session cost as out-of-network.

If you choose to use your out-of-network benefits, please verify benefit coverage prior to your first visit.

Upon your request, a “Superbill” receipt will be provided to you for each date of service to submit to your insurance company for reimbursement.

The information on the “Superbill” receipt includes the therapist’s credentials, license number, tax ID, NPI#, client information, diagnosis code, and cost of each session.
